Every country’s population tells a story – not just in numbers, but in its structure. How many children are being born? How many adults are in their prime working years? Are there more elderly people than young ones? These questions sit at the heart of age-sex composition – one of the most fundamental tools in demographic analysis. Understanding this composition is not just an academic exercise. It directly informs how governments plan schools, hospitals, pension systems, and labor markets. In short, it shapes how a society prepares for its future.

What is age-sex composition?
Age-sex composition refers to the distribution of a population by age groups and by sex – male and female. Demographers consider these two variables the most essential components of demographic analysis, since they determine a population’s economic productivity, dependency burdens, healthcare demands, and growth potential.
Age composition breaks down a population into groups – children (0-14), working-age adults (15-64), and elderly (65+). These categories reveal how many people are dependents versus productive contributors to the economy. Sex composition, on the other hand, measures the balance between males and females, usually expressed as the sex ratio – the number of males per 100 females. Biologically, slightly more males are born than females – roughly 105 boys per 100 girls – but differential mortality means sex ratios narrow over time. In many societies, social factors like son preference, gender-selective migration, and conflict further distort this balance.
The population pyramid: reading a demographic snapshot
A population pyramid is a graph that shows the distribution of ages across a population, divided down the center between male and female members. Males are plotted on the left, females on the right, with the youngest age groups at the bottom and the oldest at the top. Each horizontal bar typically represents a five-year age cohort. The resulting shape – whether a true pyramid, a column, or an inverted structure – immediately communicates the demographic character of a country.
Expansive pyramids
An expansive pyramid has both high fertility and high mortality rates among younger members, creating a sharp triangular shape. These populations have many young people but do not increase much in total number. Many countries in sub-Saharan Africa and parts of South Asia currently exhibit this pattern. They face pressure to invest heavily in education, healthcare, and employment for rapidly growing youth populations.
Stationary pyramids
A stationary pyramid has low mortality and low fertility rates, giving it a square or pillar shape. These represent stable populations that will not change significantly barring any sudden changes to fertility or mortality rates. The population is more evenly distributed across age groups, and planning demands are relatively predictable.
Constrictive pyramids
A constrictive pyramid is characterized by lower mortality with fertility rates remaining constant or declining, resulting in a population with high numbers of middle-aged and elderly people but fewer young people. Countries like Japan, Germany, and Italy show this pattern – where declining birth rates and increasing life expectancy have created an increasingly top-heavy demographic structure. These societies face acute challenges in funding pensions and elder care with a shrinking workforce.
The demographic transition and pyramid shapes
Population pyramids don’t remain static – they evolve as countries pass through stages of the demographic transition model. Stage 1 is represented by a sharply tapering pyramid sitting on a broad base, reflecting high fertility and high mortality. Stage 2 shows a reduction in mortality, especially among the youngest age groups, coupled with high fertility, so the population increases rapidly but remains young. Stage 3 has nearly vertical sides with a broad base and relatively broad peak, reflecting reduced fertility, reduced childhood mortality, and increased survival.
By Stage 4 and 5, birth rates decline sharply and the base of the pyramid narrows. More developed countries are typically found in stages three, four, and five, while the least developed countries have populations represented by pyramids in stages one and two.
Age-sex composition and development planning
Why does any of this matter beyond demographics? Because a country’s age-sex structure directly determines what it needs – and when. By examining a population pyramid, planners can anticipate demographic changes 5, 10, or 20 years into the future with reasonable accuracy, as the future adult population already exists in younger cohorts. This makes the pyramid an indispensable planning tool across multiple sectors.
Education
A large youth population creates immediate pressure on educational systems. Governments must invest in schools, universities, and vocational training to absorb growing numbers of learners. Education systems must adapt to changing demographic realities – aging societies need fewer primary schools but more adult education programs. Gender disparities in education also become visible through age-sex data, particularly where female enrollment lags behind male enrollment in certain age cohorts.
Healthcare
A youthful population demands maternal health services, pediatric care, and vaccination programs. An aging population, by contrast, shifts demand toward chronic disease management, geriatric care, and long-term support facilities. Between 1974 and 2024, the worldwide share of people aged 65 almost doubled – from 5.5 per cent to 10.3 per cent – and this number is projected to double again by 2074. This demographic shift means healthcare systems in many countries must fundamentally restructure their priorities and budgets. Sex composition matters here too: women generally live longer than men, meaning elderly care facilities disproportionately serve female populations.
Labor markets
The working-age population (15-64) forms the productive core of any economy. When this group is large relative to dependents, a country can experience what economists call the demographic dividend. While a relative increase in the working-age population lasts, it offers an opportunity for accelerated economic growth. East Asian countries like South Korea, Singapore, and China leveraged this dividend from the 1960s onward, fueling decades of rapid growth.
But the dividend is temporary. Today, population ageing is often viewed as threatening the financial viability of pensions and healthcare systems, and can result in shortages of skills and workers in the labor market. Countries with constrictive pyramids must address labor shortfalls through immigration policy, automation, and encouraging greater female workforce participation.
Sex composition shapes labor dynamics significantly. Women constitute about half of the world’s population but have lower labor force participation rates than men, and these gender gaps widen significantly when considering the presence of young children in the household. Addressing these gaps is not just an equity issue – it’s an economic imperative for countries facing demographic headwinds.
When pyramids reveal social disruptions
Population pyramids can also reveal social and historical events embedded in a country’s demographic record. Sudden indents in specific cohorts often signal the effects of war, famine, or epidemic. Bulges in the slope indicate a high rate of immigration or a preceding baby boom, while sudden indents indicate higher death rates than normal – possibly because of war, famine, disease, or natural catastrophe.
A youth bulge – where a disproportionately large share of the population falls in the 15-29 age range – presents its own set of policy challenges. A large population of adolescents entering the labor force strains the economy and polity, creating unemployment and alienation unless new opportunities are created quickly enough. The Middle East and North Africa have experienced pronounced youth bulges in recent decades, with nearly 27 percent of the region’s population between the ages of 15 and 29, creating significant pressure on governments to generate employment and maintain social stability.
Sex ratio imbalances and their consequences
When sex ratios deviate significantly from natural norms, the consequences ripple across social and economic systems. In several countries, particularly in South and East Asia, deeply entrenched son preference has skewed child sex ratios through sex-selective practices. As India undergoes demographic transition, policymakers must consider how historical sex ratio imbalances will shape future needs – today’s missing girls will mean fewer working-age women in coming decades, affecting labor markets, care economies, and social structures. India’s Pre-Conception and Pre-Natal Diagnostic Techniques (PCPNDT) Act of 1994 was a direct policy response to this demographic distortion.
At the other end of the age spectrum, women consistently outnumber men among the elderly due to longer female life expectancy, requiring healthcare and pension policies designed with this gender distribution in mind.
From data to policy: why age-sex composition matters
The UN’s World Population Ageing 2023 report makes clear that countries need to act ahead of demographic shifts – not after. Early-transition countries should prioritize reproductive health services, education, and job creation to capture the demographic dividend. Mid-transition countries should begin building social protection and healthcare systems for an aging future population. Countries experiencing population aging typically address related challenges through a combination of policies: increasing retirement ages, encouraging higher labor force participation, reforming pension systems, supporting family-friendly policies to increase fertility, and in some cases, actively recruiting immigrants to address labor shortages.
The dependency ratio – which measures the proportion of non-working age people (children and elderly) relative to the working-age population – is one of the key metrics derived from age-sex data. A high dependency ratio signals fiscal stress; a low one signals a potential growth window. Both conditions demand forward-looking governance.
Demographic data, particularly age-sex composition, is not just statistical trivia. It is the foundation upon which education ministries plan school construction, health departments allocate budgets, labor ministries forecast shortfalls, and finance ministries model pension sustainability. Countries that read their population pyramids carefully – and act on what they see – are far better positioned to manage the demands of their present and invest wisely in their future.
